米国編 COPD—米国における外来でのCOPD治療の実際

◎COPDの重症度は1秒量のみでは決まらない．
◎治療は薬物療法と同様に非薬物療法も重要である．
◎薬物療法の基本は吸入気管支拡張薬であるが，重症度に応じて吸入ステロイド（ICS）や経口の気管支拡張薬，マクロライドなどの併用も組み合わせる．
◎禁煙指導は短時間でも効果があり，無理だと諦めさせず声をかけ続けるべきである．また，呼吸器リハビリテーションは忘れられがちではあるが重要である．
◎酸素療法は適応にも留意する．日中の酸素飽和度がCMS（Center for Medical and Medicaid Services）の基準に満たなくても，運動時や夜間の低酸素血症が隠れている場合がある．
