便秘・消化管閉塞・腹水

Point
◎便秘は入院患者で多く，エンド・オブ・ライフ（EOL）期には複合的な要因により長期化しやすい．
◎悪性腸閉塞は予後不良で，手術は予後60日以上が見込まれる場合に適応となる．除圧のための短期間のドレナージと薬物療法が基本である．
◎腹水にはがん・非がんを問わずさまざまな原因があり，血清-腹水アルブミン勾配（SAAG）および腹水中総蛋白質値による分類が診断・治療に有用である．
◎EOL期の腸閉塞および腹水貯留では，500〜1,000mL/日への輸液の減量が苦痛の除去につながる．
