下大静脈評価の最前線

Point
◎下大静脈（IVC）の評価は循環血液量（fluid status），輸液反応性（fluid responsiveness）に分けて考える．
◎fluid responsivenessの評価には，静的指標だけでなく，動的指標も併用する．
◎IVCの評価として，IVC径，collapsibility index（cIVC），distensibility index（dIVC）を計測する．
◎IVCの測定に影響を与えるさまざまな要因に注意する（陽圧換気か自発呼吸か，体勢，胸腔内圧，右心不全の有無など）．
