収縮能が低下している心不全（HFrEF）の治療を教えてください

Question 1
収縮不全型心不全ではACE阻害薬，β遮断薬を必ず開始しなければなりませんか？　開始してはいけないときはありますか？
　アンジオテンシン変換酵素（ACE）阻害薬とβ遮断薬は，収縮不全型心不全（heart failure with reduced ejection fraction：HFrEF）治療のなかで，長期予後に関して最もエビデンスがある薬剤である．可能な限りHFrEFではこれらの薬剤を開始する必要があると考えられるが，状況によっては開始を控えるべき場合もある．
