収縮能が保持された心不全（HFpEF）の治療はどうすればよいですか？

心収縮力が保たれた心不全（HFpEF）があると聞きましたが，その病態はどういうものですか？
心不全の定義とHFpEF，HFrEF
　心不全は，「慢性の心筋障害により心臓のポンプ機能が低下し，末梢主要臓器の酸素需要に見合うだけの血液量を，絶対的にまた相対的に拍出できない状態」と定義されている．つまりその主体は，収縮能の低下を主体とした血行動態的な疾患群と考えられてきた．
　しかし，収縮能の代表的な指標である左室駆出率（left ventricular ejection fraction：LVEF）で評価すると，収縮能が保持された心不全（heart failure with preserved ejection fraction：HFpEF）の頻度は年々増加傾向にあり，収縮能が低下した心不全（heart failure with reduced ejection fraction：HFrEF）と同等の頻度であることが報告された^1）．さらにこの報告では，HFpEFの予後がHFrEFの予後と同様に悪いということも報告され，HFpEFの病態や治療がこれまで以上に注目されることとなった．
