発熱と頭痛

Point
◎発熱＋頭痛の鑑別疾患としてウイルス感染，髄膜炎（細菌性・結核性・真菌性），硬膜外膿瘍，感染性心内膜炎といった感染症を疑う．
◎非感染症としては，薬剤性髄膜炎，癌浸潤，Behçet病，全身性エリテマトーデスなどの可能性が考慮される．
◎腰椎穿刺を安全に行うには頭蓋内病変の確認が必要であり，頭部CTは有用であるが時間がかかるため，その適応は慎重に見極める．
