敗血症の電解質異常と輸液療法

Point
◎早期目標達成指向型管理法（EGDT）は，最新ガイドラインでは推奨されなくなった．しかし，EGDTから「初期輸液の有用性」と「過剰輸液の弊害」という知見が得られた．
◎初期輸液：細胞外液補充液 30mL/kg以上を3時間以内で投与する．
◎輸液反応性は，静的指標ではなく動的指標で評価する．
◎最終的には過剰となった体液量を正常化する．
◎輸液は循環動態維持の一手段にすぎない．カテコラミン，アルブミン，輸血などもまとめて理解することが重要である．
