胸焼け—胃食道逆流症（GERD）

Point
◎胃食道逆流症（GERD）は胃食道逆流（GER）により引き起こされる食道粘膜傷害と煩わしい症状のいずれかまたは両者を引き起こす疾患と定義されている^1）．
◎日本では，食生活の欧米化やHelicobacter pylori感染率の低下，除菌治療の普及に伴う日本人の相対的な胃酸分泌の増加に伴い^2），GERDは増加している^3）．
◎GERD診療ガイドラインでは，初期診療の第一選択としてプロトンポンプ阻害薬（PPI）の使用が推奨されている^1）．
◎近年，標準量のPPIを8週間内服しても食道粘膜傷害が治癒しない，またはGER由来と考えられる症状が十分に改善しないPPI抵抗性GERDも多いことが明らかになってきた^4）．
◎日本では2015年に従来のPPIより酸分泌抑制効果が強力なカルシウム競合型アシッドブロッカー（P-CAB）であるボノプラザン（タケキャブ^®）が発売された．
