自己抗体検査—何を・いつ・何のためにオーダーするのか？

Point
◎抗核抗体の検査は，まず間接蛍光抗体法（IF法）でオーダーすべきである．
◎全身性エリテマトーデス，全身性強皮症，皮膚筋炎/多発筋炎，混合性結合組織病，Sjögren症候群を疑ったときに抗核抗体をオーダーする．
◎「抗核抗体陽性＝膠原病」ではないし，逆に「抗核抗体陰性＝膠原病ではない」ともならない．
◎自己抗体の力価は，必ずしも疾患活動性を反映しない．
◎膠原病の診断には用いるが，あくまで臨床診断が重要である．
