Point
◎高齢者は加齢の進行に伴い潜在的に不適切な薬剤による薬物有害事象のリスクが高くなるため，適宜処方の見直しを行うことが重要である．
◎潜在的に不適切な薬剤のスクリーニングツールとして「Beersクライテリア」，「STOPP/STARTクライテリア」，「高齢者の安全な薬物療法ガイドライン」などがある．
◎減薬プロトコールは，個々の患者の臨床背景を考慮することで減薬の適切性を判断できるツールである．
◎処方適正化を推進するためには，複合的なツールの使用や多職種連携による取り組みも重要である．
