◎調剤，医薬品の供給に加え，処方箋内容の確認とその内容に関する疑義照会は薬剤師の重要な役割である．
◎保険薬局では，内服薬のアドヒアランス状況や健康食品，サプリメントの使用に関する情報など，即時性は低いものの診療上重要な情報について，トレーシングレポートを作成し医療機関に送付している．
◎病院入院時の持参薬確認はポリファーマシーを整理する重要な機会である．
◎薬剤師の教育的介入により，高齢者の不適切処方が削減できる可能性がある．
◎不適切処方の削減に加え臨床アウトカムを改善するためには，薬剤レビューだけでなく多職種で連携した多面的な介入が必要である．
