抗凝固療法中の出血高リスクとその対策

Point
◎安全かつ有効な抗凝固療法には，抗凝固薬の適正使用が最も重要である．
◎抗凝固療法中の出血性合併症のリスク因子として，高齢，低体重，貧血，腎機能障害，抗血小板薬との併用，出血の既往などが挙げられる．
◎頭蓋内出血の危険予測には頭部MRI，消化管出血の危険予測には消化管内視鏡検査が有用である．
◎重篤な出血の発症時には，バイタルサインの安定化，止血，抗凝固薬の休薬に加え，抗凝固薬中和薬の投与を検討する．
◎抗凝固薬の特異的中和薬として，ワルファリンにはプロトロンビン複合体とビタミンK，ダビガトランにはイダルシズマブがある．
