◎処方されている薬の適応疾患か，有害事象の発生がないかどうか今一度確認する．
◎入院中に適切に処方内容を見直し，その変更内容を患者のみならず処方医にもフィードバックする．
◎処方医にフィードバックする際には，相手の行動変容を促せるように注意する．
