気管支喘息の安定期に用いられる薬剤と使いかた

Point
◎喘息治療の長期管理薬は吸入ステロイド薬（ICS）が基本であり，発症早期から用い，少量でも継続することが奨励されている．
◎呼吸機能が低下していたり，症状が強い場合にはICS/LABA（長時間作用型β₂刺激薬）が勧められる．
◎長時間作用型抗コリン薬（LAMA）は，2018年のガイドライン改訂以降，治療ステップ2から使用が可能となった．
◎治療ステップ1から使用可能なロイコトリエン受容体拮抗薬（LTRA）はLABAに比べ気管拡張作用は若干劣るが，ICSに次ぐ抗炎症作用や直接的なリモデリング抑制作用などがある．
◎妊婦の喘息発作は，流産，胎児発育不全・脳障害の危険因子となるため，普段よりも厳格な管理が必要となる．
