肺高血圧症の診断と治療

Point
◎肺高血圧症は原因不明の持続的な肺動脈圧の上昇により，右心不全へと進行する予後不良の難治性疾患である．従来，平均肺動脈圧が25 mmHg以上と定義されていたが，最新の第6回ニースシンポジウムにて診断基準が20 mmHg以上へ引き下げられる方向で議論が行われている．
◎肺高血圧症は病変の存在部位により第1〜5群に分類され，病因によりさらに詳細に分類されている．
◎肺高血圧症は，専門施設における右心カテーテルを中心とした早期診断と病型に基づく治療選択が最も重要である．
