抗凝固のジレンマをどう解決するか

Point
◎非弁膜症性心房細動では血栓塞栓症と出血リスクを評価し，抗凝固療法のリスクとベネフィットを考慮したうえで治療法を決定する．
◎血栓塞栓症高リスク患者は出血高リスク患者でもあり，出血リスクが高いことを理由に抗凝固療法を行わないのは慎むべきと考えられる．
◎血栓塞栓症と出血リスクがきわめて高い場合，左心耳閉鎖が良い選択肢となり得る．
