TAVIとMitraClip^®の適切な適応とは？

Point
◎現在，日本では外科中等度リスクまでが経カテーテル大動脈弁留置術（TAVI）の適応であるが，今後は欧米と同様に手術低リスク患者にも適応が拡大することが予想される．
◎標準的薬物治療後も心不全症状が残存する機能性僧帽弁逆流症患者に対するMitraClip^®治療は患者の予後を改善する報告があり，治療の選択肢として考慮される．
◎TAVIとMitraClip^®の適切な適応は拡大傾向にあるが，未解決な問題は多い．今後の多施設共同研究などの結果，長期成績も考慮し，時代背景に応じた適応判断が重要である．
