アルコール問題は頻度が高く，日本では生涯でアルコール依存と診断される患者が107万人と推計されています1）．アルコール問題には図1のような各段階があります2）．アメリカ精神医学会の診断基準であるDSM-5では，アルコール依存とアルコール乱用を合わせて「アルコール使用障害（alcohol-use disorder）」となりました．またproblem drinkingはアルコールによる健康上の問題が生じている状態です．注目されやすいのはアルコール使用障害ですが，重要なのはそこまで至らないrisky useの段階から介入することです．risky useは見逃されがちですが，危険なproblem drinkingやalcohol-use disorderに進行する可能性もありますし，多くのアルコール関連問題（喧嘩や自殺での入院など）はアルコール量や泥酔の頻度が低い群でも起こっていたという報告もあります3）．risky useの定義は「65歳以下の男性では＞14単位/week or 4単位/日，女性もしくは65歳以上では消費アルコール量が＞7単位/week or ＞3単位/日」となっています．
有名なCAGE questionはアルコール依存に対しては有用ですが，より早期の危険な飲酒（risky use）の検出には劣るとされます4）．unhealthy useを疑ったときはAUDIT（アルコール依存症スクリーニングテスト）もしくは時間がなければAUDIT-C（AUDITの簡易版）を使いましょう．最近は計算に便利なアプリやWebサイトもあります．
