総論
脳梗塞は原因により主にアテローム血栓性梗塞，心原性脳塞栓症，ラクナ梗塞に分かれるが，通常精査（MRI，頸動脈/心臓超音波検査，Holter心電図など）を行った後に10〜40％ほどは原因不明（cryptogenic stroke）に分類される1）．この原因には表12）のようなものがあるが，若年（18〜30歳）では動脈解離・血栓傾向・先天性心疾患が多く，年齢が上がるごとに若年性の動脈硬化や後天性構造的心疾患が増え，60歳以上になると検査で捕まえられない心房細動が多くなる．これらの原因から重要なものを紹介する．
