Point
◎ANCA関連血管炎は多発血管炎性肉芽腫症（GPA），顕微鏡的多発血管炎（MPA），好酸球性多発血管炎性肉芽腫症（EGPA）の3つを含む疾患概念であり，中〜小型の壊死性血管炎である．
◎GPAでは中〜小型の壊死性血管炎に加えて，血管外の壊死性肉芽腫性炎症が特徴的であり，EGPAは中〜小型の壊死性血管炎に加えて，好酸球性の炎症を伴うことが特徴的である．
◎ANCA関連血管炎の診断においては，同様の症状を呈するような感染症の除外が重要となってくる．
◎GPAとMPAは，同様の薬剤を用いて治療をされるのに対して，EGPAはこれら2疾患とは異なる薬剤を用いて治療することもある．
◎GPA/MPAに対してC5a受容体拮抗薬は効果が期待でき，今後ステロイドを用いない治療もできるようになってくる可能性がある．
