緑膿菌薬—どんなときにカバーし，どう治療する？

Point
◎水系曝露，医療曝露が想定される背景では，原因微生物の1つとして緑膿菌を考える．
◎緑膿菌カバーの要否は，患者背景，感染巣，重症度を十分評価し，可能な限り培養提出検体のグラム染色所見を確認する．
◎自施設のアンチバイオグラムを踏まえ，緑膿菌を含む想定される原因微生物を過不足なくカバーする（経験的治療）．
◎耐性緑膿菌の発生リスクを考慮し，想定する臓器・感受性・既知のアレルギーと臓器障害などの観点で問題がなければ，抗緑膿菌作用をもつペニシリン系，セファロスポリン系抗菌薬の使用から検討する（最適治療）．
