Case 1
糖尿病を指摘されるも特に通院歴のない72歳女性
　3日前から倦怠感を自覚し，前日は食欲もなく飲水程度だった．前医を受診し第三世代の経口セファロスポリン系抗菌薬を処方されるも症状の改善は乏しかった．来院当日，発熱あり体動困難となったために救急要請をした．救急隊現着時のバイタルサインは体温39.2℃，血圧94/62 mmHg，脈拍118回/分，SpO₂ 88％（room air），呼吸数20/分だった．敗血症が疑われて，救急救命センターへ搬送となった．身体所見にて肋骨脊柱角の叩打痛が陽性で，尿グラム染色では白血球に貪食された腸内細菌様のグラム陰性桿菌を認めた．また，静脈血での血液ガス分析結果はpH 7.146，糖589 mg/dLだった．以上より，腎盂腎炎による糖尿病性ケトアシドーシスを合併した敗血症として対応をするもバイタルサインは安定せず，逆にショックとなり，ショック状態が遷延した（表1）．
