ステージDと思ったらすべきことは？

Point
◎ステージD心不全は，すべての治療が無効となった末期心不全である．
◎ステージD心不全かも？　と思ったときは，まずはステージA〜Cの治療（GDMT）が十分に実施されているか，アドヒアランスは問題ないかを確認する．
◎「本当の」ステージD心不全患者であれば，心臓移植の適応がないか検討する．
◎心臓移植の適応がないステージD心不全患者には，advance care planning（ACP），緩和ケア，ケアのコーディネーションなど総合内科的視点が重要である．
