Point
◎心不全手帳は患者のセルフケア教育に便利なツールである．心不全は進行性の予後不良な病気である一方，セルフケアで再入院を防げることを患者に説明しよう．
◎看護師（体重などのセルフ・モニタリング指導），薬剤師（服薬指導），栄養士（栄養指導），理学療法士（運動療法），かかりつけ医（予防接種などのヘルスメンテナンス）にはそれぞれの役割がある．多職種で協力して患者のセルフケアを支援しよう．
◎心不全が悪化している徴候とそれに気づいたときの対処法＝アクションプランを説明しておこう．
