緊張型頭痛の診断と治療

Point
◎緊張型頭痛は頭蓋周囲の圧痛を伴う場合が多い．
◎片頭痛との鑑別は困難な場合が多く，片頭痛の予兆期に肩こりを伴うため緊張型頭痛と誤診しやすい．特に慢性片頭痛では頭痛の性状が緊張型頭痛様となる点にも注意する．
◎頻度が低い場合は頓用鎮痛薬，頻度が高い場合には少量のアミトリプチリンによる予防療法が第一選択となる．難治の場合には，非薬物療法を含めた多角的アプローチが推奨されている．
