群発頭痛の診断と治療

Point
◎群発頭痛は，一側性の眼窩部を中心とした極めて重度の頭痛で，発作時に同側に結膜充血や流涙などの頭部自律神経症状を伴うことが特徴である．
◎群発頭痛は，数週間〜数カ月間にわたる群発期を有し，群発期には1回/2日〜8回/日の頻度で，15〜180分間の頭痛発作を認める．
◎群発頭痛では，片頭痛と異なり頭痛発作時に，痛みのために落ち着きなく歩き回ったり，頭や体を動かしたりすることが多い．
◎群発頭痛の急性期治療としてスマトリプタン皮下注射と高濃度酸素吸入が有効である．
◎群発頭痛群発期の予防療法としてベラパミル内服や，ステロイド短期間服用がある．
