経口CGRP受容体拮抗薬（ゲパント系薬剤：gepant）

ゲパント系薬剤とは
長年，頭痛研究において，頭痛・疼痛と関連しているペプチドとしてカルシトニン遺伝子関連ペプチド（CGRP）が知られてきた．2021年に，このCGRPやその受容体を阻害する抗CGRP関連抗体薬として，ガルカネズマブ，フレマネズマブ，エレヌマブが日本で発売された．近年，この抗CGRP関連抗体薬の治療効果から，CGRP受容体の拮抗薬である低分子の経口CGRP受容体拮抗薬・ゲパント系薬剤にも，注目が集まっている．
いままでの基礎的な研究によって，ゲパント系薬剤はCGRPに特異的に拮抗する低分子薬剤として使用されてきた1）．これら薬剤は，既存の抗体薬と異なり，低分子であるため内服や点鼻薬として使用できることが特徴である．また，その持続時間から急性期治療薬だけてなく，予防薬としての作用がある点も注目されている．また，最近の研究からゲパント系薬剤はCGRP受容体に対する拮抗作用により効果を発現すると考えられているが，細胞系の研究において，アミリンに高い親和性のあるAMY1受容体にも作用する報告がある2〜4）．
