脳血管障害と頭痛—walk-in SAH，動脈解離，脳静脈血栓症

Point
◎頭痛診療では，くも膜下出血（SAH）などの緊急性の高い二次性頭痛を鑑別することが第一歩である．二次性頭痛の鑑別は問診，身体所見，神経学的所見を丁寧にチェックし，画像検査や血液検査，髄液検査などを組み合わせて進める．
◎SAHによる頭痛は，典型的な雷鳴頭痛は約80％であり，軽症例では歩いて来院するwalk in SAHも珍しくない．軽症例ほど誤診率が高く，最も多い特異的な誤診は片頭痛である．
◎頸部動脈解離と脳静脈血栓症では頭痛が唯一の症状のことがあり，特異的な性状もないので，新規の頭痛が持続する場合には，これらの疾患も鑑別疾患に挙げ，詳細な病歴聴取や画像診断が必要である．
