十二指腸癌（十二指腸腫瘍）

　ひと昔前に「十二指腸癌」と言ったら，概ねそのイメージは決まっており，あまり見ない癌，よくわからないけれども治療がややこしい病気という印象だった．ようするに，希少癌であり，あまり注目されてはいなかったのである．そもそも十二指腸の疾患自体が重要視されるような時代ではなく，上部消化管内視鏡時に十二指腸の観察はそれほど力を入れなくていいよ，と教えられた内視鏡医がまだ多いはずである．実際に，胃では定型的な観察法は多く存在するものの，十二指腸に関しては定型的な観察法すら確立していないのが現状である．しかしここへ来て，十二指腸の上皮性腫瘍の発見率が明らかに増加している^1〜3）．その原因やリスクファクターなども明らかになっていないが，内視鏡機器の進歩や内視鏡医の関心の高まりに加え，ヘリコバクターピロリ陰性時代の到来が関連していることが考えられている．2021年には国内で初めて『十二指腸癌診療ガイドライン』が発刊され^4），いよいよ十二指腸腫瘍を避けては通れない時代がやってきている．本稿では，このあまり馴染みのない十二指腸上皮性腫瘍の種類や治療法につき概説する．
