特集 10年前の常識は非常識!?—イマドキ消化器診療にアップデート 疾患

大腸癌

　日本人の2人に1人が患うと言われる悪性新生物のなかでも，大腸癌の発生頻度は高く，年齢調整罹患率は男性では肺癌，胃癌，前立腺癌に次ぐ第4位，女性では乳癌に次ぐ第2位であり，死亡率は男性では肺癌に次ぐ第2位，女性では第1位となっている．筆者が大腸癌診療に関わるようになった25年前には，使用できる抗がん剤はフルオロウラシルとマイトマイシンのみで，切除不能がんにおける平均余命は約1年とされていた．近年，内視鏡外科手術の進歩，抗がん剤および分子標的薬，免疫治療といった治療の進歩は著しく，ステージⅡ/Ⅲ結腸癌の5年全生存率は90％に近づき，切除不能進行再発大腸癌の全生存率の中央値は約3年に迫っている．
　大腸癌の標準治療は大腸癌研究会で作成された『大腸癌治療ガイドライン2022年版』にまとめられており^1），約2年に一度のアップデートがされている．しかし，治療は日進月歩で進歩しており，昨日までの常識が今日は非常識ということは常に起こりうることである．本稿では，最近の新しい常識や今後の常識となりそうなホットな話題をいくつか紹介する．
