過敏性腸症候群

　過敏性腸症候群（IBS）は繰り返す腹痛や，それに伴う便性状や排便回数の変化を伴う機能性消化管疾患である．IBSの症状トリガーにストレスが関連することが示唆されており，脳腸相関が病態の軸とされている．
　本邦でのIBSの有病率は約10％とされ，臨床現場で頻回に出会う消化器疾患の1つである．米国の有病率と大きくは変わらない．ストレス反応に関連する副腎皮質刺激ホルモン放出ホルモン（CRH，視床下部）が消化管の運動や感覚亢進を引き起こすことがわかってきた．さらに近年IBSのコホート調査や，粘膜微小炎症や腸内細菌の研究なども進み，感染性腸炎後にIBSに進展するという感染性腸炎後IBS（post-infectious IBS：PI-IBS）という病態も知られてきた．
