特集 10年前の常識は非常識!?—イマドキ消化器診療にアップデート 疾患

　本邦における胆道癌の罹患数は，がん全体で13位，死亡数では6位のがん腫^1）であり，各種がん腫のなかで比較的稀な難治性がんとして位置づけられる．胆道癌は肝内胆管癌，肝外胆管癌，十二指腸乳頭部癌，胆囊癌に分類される．近年，2019年に『胆道癌診療ガイドライン第3版』が発刊され，2021年には『胆道癌取扱い規約第7版』発刊のほか，『十二指腸乳頭腫瘍に対する内視鏡的乳頭切除術ガイドライン』の新たな作成が行われた．そのほか，肝内胆管癌に対しては，日本肝癌研究会を中心として『肝内胆管癌ガイドライン2021年版』が作成された．本稿ではこれらのガイドラインや取扱い規約の内容を基本としながら，現在まで明らかになった知見を概説する．
