周術期にワルファリンやDOACはどうする？

Point
◎観血的手技のリスク分類を基に，個々の患者背景を考慮し，周術期管理の方針を決定する．
◎直達止血可能な手技では，適切な局所止血処置を行うとともに，原則として抗凝固療法を継続する．
◎ワルファリンを継続する場合，周術期PT-INRを至適治療域に管理することが重要である．
◎DOACを継続する場合，血中濃度がピークの時間帯を避けて手技を行う．
◎休薬を要する場合，休薬期間を最短とし，ヘパリンブリッジ（代替療法）は原則行わない．
