リズムコントロール治療はどうする？

Point
◎リズムコントロールの最大の意義は症状緩和であるが，早期介入による予後改善効果が報告されている．
◎リズムコントロールの対象は，有症候性の発作性心房細動が基本であるが，無自覚に運動耐容能が低下しているケースもあり，一見無症候性であっても洞調律化を試みる意義はある．
◎リズムコントロールの手段としては，明らかな基礎疾患がないケースではNaチャネル遮断薬が第一選択となるが，器質的疾患があるケースや持続性心房細動ではアミオダロン，べプリジル，ソタロール（保険適用外）が選択肢となりうる．
◎薬物によるリズムコントロールの効果は限定的であり，洞調律維持効果はアブレーションが圧倒的に優れている．
◎抗不整脈薬投与の際，安全性の確保が重要であり，心臓性副作用・心外性副作用に注意を要する．
