脂質異常症の一次予防としての薬物療法はどんな患者にするべきですか？

Point
◎一次予防としての脂質異常症の治療目的は，心血管疾患（CVD）発症を予防し，生命予後を改善することである．
◎一次予防は非薬物療法が中心であり，薬物療法の一次予防効果は限定的である．
◎家族性高コレステロール血症（FH）は早期診断・早期治療が生命予後改善に重要である．
◎生命予後改善を目的とした一次予防の場合，FHや糖尿病，慢性腎臓病，末梢動脈疾患がない患者において，60〜79歳の男性で，10年間のCVD発症リスクが20％を超える場合に，スタチン療法を考慮する．
◎最終的には，shared decision making（SDM）を実践して治療方針を決定する．
