　わが国では脳梗塞急性期に広く用いられているヘパリンであるが，そのエビデンスレベルは高くなく，欧米ではヘパリンの急性期投与は推奨されていない．本稿では，心房細動による心原性脳塞栓症の急性期にヘパリンによる橋渡し治療（ヘパリンブリッジ）を介さずに直接作用型経口抗凝固薬（direct oral anticoagulant：DOAC）を早期に開始することを推奨する立場から，その根拠について概説する．
