Con　CAVE高スコアでも急性症候性発作がなければ予防投与はしない

　近年，高齢化の進展，脳卒中急性期治療の進歩による死亡率の改善により，脳卒中生存者が急増している．脳卒中後てんかんは脳卒中生存者の合併症のなかで重要な疾患であり，多くの研究がなされるようになった．脳卒中後てんかん発症リスクに関する報告も蓄積されるようになり，脳卒中後てんかん発症リスクのスコアモデルが構築された（頭蓋内出血による脳卒中後てんかん発症リスクモデル：CAVEスコア，脳梗塞による脳卒中後てんかん発症リスクモデル：SeLECTスコア）．これらのスコアを用いれば脳卒中後のてんかん発症の予測が可能となり，CAVEスコアでは3点で約35％，4点で46％が脳卒中発症後7年間にてんかんを発症することが判明している．SeLECTスコアも同様であり，7点以上であれば約半数の症例がてんかん発症のリスクがある．そうしたなかで，いずれにしろ将来てんかんを発症するのであれば，抗てんかん薬をあらかじめ服用したほうがよいのではないかという機運が高まっている．この抗てんかん薬の予防投与に対して，本稿では筆者が否定的な立場にあると仮定して，現状のエビデンスを基に考察したい．
