CASE 11 片頭痛—片側性の悪心を伴う拍動性の頭痛を訴える45歳女性

45歳女性
現病歴　20代の頃から月に1回程度の頭痛があり，市販の頭痛薬を頓用して様子をみていた．頭痛は片側性の悪心を伴う拍動性の頭痛で，2日程度で消失していた．頭痛の性状は変わらないが，最近頻度が増え，月に4〜5回程度となった．閃輝暗点を疑うような前兆があり，悪心と光過敏・音過敏を伴っている．頭痛がひどいときは体動困難で仕事を休むこともあり，日常生活に支障を感じている．月経周期との関連はなく，発作時に脱力や麻痺，めまい，耳鳴り，アロディニアなどの症状は伴わない．数日前にかかりつけ医を受診し頭部MRIを施行したが，異常はなかった．ナプロキセン（ナイキサン^®）を処方されたが，頭痛頻度や症状の改善が乏しかったため，当院を受診した．
