あちこちの関節が痛い

　内科外来をしていると，問診票に「主訴：あちこちの関節が痛い」と書かれ，痛みを示す体のシェーマには首，両肩，両肘，腰，両膝，両足関節のすべてに◯がつけられているような患者に遭遇することがしばしばある．患者は関節が痛いからといって必ずしも整形外科を受診するわけではなく，「どこが悪いのかよくわからないから内科を受診する」のである．多発関節痛あるいは関節炎を呈する疾患は実に多く（表1）^{1, 2）}，幅広い視野を持って診療に当たる必要がある．本稿では，そのすべてを解説することは不可能であるため代表的な疾患のみ解説する．疾患に特異的な病歴聴取および身体診察と，効率の良い検査で診断を絞っていくことがポイントである．
