Point
◎利尿薬ブレーキ現象・post-diuretic sodium reabsorptionは生理的な反応であり，利尿薬抵抗性は臨床的概念である．
◎利尿薬抵抗性は，うっ血の悪化から予後不良につながる．
◎利尿薬抵抗性の克服には，利尿薬の薬物動態・薬物力学の理解が必要である．
◎近年，種々の新規利尿薬が使用可能となったが，うっ血・浮腫改善のためにはループ利尿薬との併用が基本である．
