抗凝固薬，抗血小板薬の減薬・中止基準

Point
◎抗血栓薬の使用は，de-escalation（段階的縮小）に向かっている．
◎出血は予後を大きく悪化させるので，抗血栓薬の使用時には十分留意する必要がある．
◎抗凝固薬投与がためらわれる高齢患者にも，安易なオフラベル（不適切）の減量は慎むべきである．
◎抗血栓薬中止のタイミングをどうするか，その際の患者との合意形成が重要である．
