緊急気管挿管時の薬剤選択

Point
◎緊急気管挿管のスタンダードは迅速導入気管挿管（rapid sequence intubation：RSI）であり，鎮静薬に続けて筋弛緩薬を投与する．
◎RSIは初回挿管成功率を上げ，かつ挿管に関連した有害事象の発生率を下げる．
◎鎮静薬は，気管挿管（特にRSI）の実施に不可欠である．短時間作用型で静脈投与が可能なミダゾラム，ケタミン，プロポフォールなどが使用できる．
◎筋弛緩薬は，RSIに際し挿管条件を最適化するために重要である．ロクロニウム，スキサメトニウムが代表的な薬剤である．
