てんかん重積状態の治療薬選択

Point
◎てんかん重積状態への対応では，発作持続時間で「早期てんかん重積状態」，「確定したてんかん重積状態」，「難治てんかん重積状態」の3ステージに分け，それぞれのステージに応じた静注抗てんかん発作薬を速やかに投与して，一刻も早いてんかん重積状態の解除を目指す．
◎てんかん重積状態は長期化するほど後遺障害のリスクを高めるため，セカンドライン治療の静注抗てんかん発作薬でてんかん発作がコントロールできない，あるいは舌根沈下・気道分泌物などにより気道閉塞が切迫している場合は，サードライン治療として速やかに全身麻酔薬を用いて鎮静し，気管挿管を行い人工呼吸器管理とする．
