利尿薬

Point
◎利尿薬は，尿細管の各セグメントにおけるナトリウムや水などを通す細胞膜にあるトンネル（チャネル/トランスポーター）に作用する．
◎体液量の是正が必要な急性心不全患者には，速やかに，かつその患者の病態に合った投与方法でループ利尿薬を開始し，治療反応性に応じ早期から別の機序の利尿薬も併用する．
◎腎の微小循環を念頭に置き，過降圧・過除水に陥らぬよう管理する．
