低ナトリウム血症

Point
◎低ナトリウム（Na）血症は自由水摂取量が排泄能力を上回ることで発症する．
◎高張食塩水による初期治療は脳浮腫や脳ヘルニアを防ぐことを目的としている．重症度（血清Naの値）と緊急度（脳浮腫を疑う症状）のみにより決定し，低Na血症をきたす病態によらない．
◎浸透圧性脱髄症候群（ODS）のリスクや発症経過から補正速度を設定する．
◎尿量や尿Na，尿カリウム（K），尿浸透圧などの所見から水利尿状態の変化を意識する．
