Point
◎重症関連コルチコステロイド障害（CIRCI）の診断において，副腎皮質刺激ホルモン（ACTH）負荷試験やランダム血漿コルチゾール測定も選択肢となるが，重症患者ではこれらの検査の確実性が低いため臨床判断が重要である．
◎難治性のショックに対するステロイド治療としてヒドロコルチゾンを1日200〜300 mg，分割投与または持続投与する方法が挙げられている．敗血症性ショックでは有効だが，軽症例での使用は推奨されない．
◎CIRCIにおいてフルドロコルチゾンの追加は一部の研究ではヒドロコルチゾン単独投与と比較して差がないことから，必ずしも推奨されていないが，一部では併用が行われている．
◎手術の種類に応じてステロイドの増量が必要とされる．特に，長期間高用量のステロイドを使用している患者は視床下部-下垂体-副腎軸（HPA axis）の抑制があるため，手術前に評価を行い適切なステロイドカバーを提供する．
