中高年の血便—出血の重症度の評価が重要

Point
◎中高年の血便は，下腹部痛を伴う疾患と伴わない疾患があることを理解し，鑑別診断を進める．
◎病歴聴取，身体診察，血液検査を通して，出血の重症度を評価する．その際は，OaklandスコアやNOBLADSスコアなどのスコアリングモデルの活用が望ましい．
◎重症度が高い患者は，急性出血下での大腸内視鏡検査を行う必要性があるため，高次医療機関への搬送が必要になる．
