中心静脈カテーテル（CVC）留置—合併症のバリエーション豊か…まさかこんな所にカテ先が！

Point
◎中心静脈カテーテル（CVC）の留置は致死的な合併症が起こり得る手技であるため，十分な準備が重要である．
◎CVCの留置前に，必要性や適応について改めてチームで考えることが必要である．
◎穿刺はout-of-plane法（短軸交差法）を用いてdynamic needle tip positioning（DNTP）で血管内に針先を進めることで，誤穿刺のリスクを減らせる．
◎誤穿刺による出血が止血困難となる可能性が高い場合は血管外科医などに相談する．
◎内頸静脈のCVCを抜去する際は，空気塞栓予防のために臥位に吸気で止めたタイミングで抜去する．
