増えている梅毒の診断・治療—ステルイズ^® の使い方を中心に

Point
◎トレポネーマ（Tp）検査陽性であれば梅毒の可能性が高いが，現在の感染と過去の感染を区別できないため，現在の疾患活動性の有無と治療効果判定には非Tp検査（主にrapid plasma reagin：RPR）を用いる．
◎神経梅毒を合併していない早期梅毒の標準治療はペニシリンGベンザチン（ステルイズ^®）240万単位を単回筋注し，後期梅毒の場合は同量を1週間空けて合計3回筋注する．
◎治療開始後，治療前値から1/4以上RPRが低下すれば治療成功と考えるが，再上昇する場合には再感染の可能性を，なかなか低下しない場合にはHIV感染症合併や神経梅毒合併の可能性を考慮する．
