Question62　71歳女性．主訴「複視」
現病歴　10年以上の病歴の2型糖尿病で，近医で経口血糖降下薬（メトホルミン2,000mg/日，テネリグリプチン20mg/日）およびインスリン（インスリンアスパルト各食直前4単位，インスリングラルギン眠前6単位）を処方されていたが，血糖コントロールは目標未達成が続いていた．来院4日前，起床時に複視を自覚したため近医を受診し，脳神経外科に紹介され頭部MRIを撮像されたが動脈瘤，脳血管障害の所見はなく，当院眼科を紹介受診した．血糖コントロール目標未達成であったため，当科紹介受診となった．
身体所見　身長154cm，体重62.9kg．脈拍86/分．血圧146/84mmHg．右眼瞼下垂，右方視による複視（右眼内転障害）を認めた．舌偏位，顔面神経麻痺，構音障害は認めず，Barré徴候，finger-nose試験に異常を認めなかった．
